Tara Marshall is a healthcare transformation lead at These Hands Academy, working at the intersection of digital health, implementation, and compassionate community care. Over the past three years, Tara has supported the development and scale implementation of the Connected Care programme across health and social care settings, helping organisations move beyond technology adoption toward meaningful system transformation.
As the NHS continues to evolve towards neighbourhood-based models of care and the three key shifts in healthcare delivery, Tara has worked closely with care providers, clinicians, communities, and system partners to translate digital innovation into practical, relationship-led approaches that improve communication, confidence, and outcomes for frontline teams.
Now internationally recognised, the Connected Care programme has demonstrated how simple, direct, and impactful interventions can help join together fragmented systems of care. This session will offer honest reflections on the realities of implementation, the challenges of partnership working across complex systems, and the lessons learned from embedding sustainable change at scale.
